2.) Your "ship to" address, including your street address, city, state, zip code, and country (if outside of the USA).
3.) How many books you would like. To whom you would like them autographed, with the exact spellings. Any other "special" signing instructions.
4.) Shipping and handling, per book or per T-shirt: $5.00 for the slower "media mail" rate or $6.00 for the standard 3 day delivery of "1st Class/Priority Mail" via the US Postal Service.
5.) How many T-shirts you would like. The sizes of the T-shirts.
